Netcare 911 responded to reports of a collision on the R546 near Charl Cilliers.
Reports from the scene indicate that two light motor vehicles were involved in a collision.
Paramedics assessed the scene and found that four occupants from one of the cars sustained fatal injuries and were declared deceased on the scene.
An adult male from the second car sustained moderate injuries.
Once stabilised the patient was transported by ambulance to hospital for further treatment.
All necessary authorities were on scene.
